We are so sorry to see you go. We understand that every student deals with their own set of unique circumstances. 

Please let us know if there is any way we can help you continue your lessons. If you are going on an extended break, we recommend that you fill out our Vacation form here instead of this drop form. 

If you would still like to drop, please fill out the adjacent form. We require a 2-week notice so we can inform the teacher and they can help you unwind your lessons. Refunds will be calculated based on the date you submit this form and the next two classes as per school policy.
